Non-Resident Application for Enrollment
Beginning the 2017-2018 school year, USD 508 will no longer be mailing out enrollment packets. Click the links below to download and print the forms necessary to enroll your student. If you do not have access to print these forms, they can be printed for you at the Board of Education Office located at 1108 Military.
