Free Physicals Offered for 2011-2012 School Year
The 2011-2012 school year is almost upon us and many of your children are planning on participating in sports. Since physicals are required for student athletes, Freeman Sports Medicine is offering free physicals Tuesday, August 2 at Freeman Sports Medicine Center, 1111 McIntosh Circle Drive. Middle school physicals will be at 6:15 p.m. with students leaving the middle school at 5:45 p.m. High school physicals will be at 6:00 p.m. with students leaving the high school at 5:30 p.m.
Due to the large number of athletes that are seen for physicals, it is important to follow a few guidelines to make sure your child's physical runs smoothly and in a timely manner.
- Please show up at your school's assigned time.
- Please wear shorts and a T-shirt. (No mini-skirts please.)
- If you are under 18, you must have your parent's signature on the physical form. You will not be allowed to complete the physical without your parent's signature on the form.
- A representative from your school will be responsible to ensure that proper parental consent is obtained before you are allowed to participate.
